The Transition process

If you child has a Statement of Educational Needs, at his/her Annual Review in Year 9, the school will draw up a transition plan, with a view to helping your son/daughter to prepare for the move from school to adult life. The transition plan should be part of future Annual Reviews.

The Transition Plan should include the following:

  • Curriculum needs during transition from school to post 16 provision
  • Consideration of any new professionals/agencies who will need to be involved
  • Future input/support from the Health Authority and Social Services
  • The transfer of relevant information to any adult support services
  • The use of technological aids
  • Post 16 options
  • Views of parents in relation to transition
  • Views of your son/daughter

The following people should be invited to the Transition review:

  • You, as parents
  • Your son/daughter
  • School Staff
  • Social Services
  • Connexions Services
  • Health Authority (where necessary)

The L.E.A must consult you before transferring any information, including a copy of the Statement, to a College of Further Education.
