The Transition process
If you child has a Statement of Educational Needs, at his/her Annual Review in Year 9, the school will draw up a transition plan, with a view to helping your son/daughter to prepare for the move from school to adult life. The transition plan should be part of future Annual Reviews.
The Transition Plan should include the following:
- Curriculum needs during transition from school to post 16 provision
- Consideration of any new professionals/agencies who will need to be involved
- Future input/support from the Health Authority and Social Services
- The transfer of relevant information to any adult support services
- The use of technological aids
- Post 16 options
- Views of parents in relation to transition
- Views of your son/daughter
The following people should be invited to the Transition review:
- You, as parents
- Your son/daughter
- School Staff
- Social Services
- Connexions Services
- Health Authority (where necessary)
The L.E.A must consult you before transferring any information, including a copy of the Statement, to a College of Further Education.
